At this time, van Beek Family Dentistry is not accepting new patients. As soon as our wait list has decreased, we will be glad to start accepting again! As a new patient, the first visit to our office is designed to get you better acquainted with all we offer as well as introduce you to Dr. vanBeek or Dr. Madhu and our caring staff. We encourage questions and always do our best to deliver quality care.

Please take a moment prior to your scheduled appointment to print our “New Patient Registration Form” (Selecting Children or Adult) below. We ask that you complete the form and bring it with you to your appointment or email it to our office at drvanbeek@hurontel.on.ca so we may better assist you in a timely manner.

If you have any x-rays from another dental office that you wish to have forwarded to our office please fill out the “X-ray Request Form” below and either email it to our Office Staff at drvanbeek@hurontel.on.ca or bring it directly to the office.

Once every 5 years, we are required by law to have our patients update their full medical chart with the below “Medical Health Update” (Children or Adult) form.
